I don't know why the go people thought this was a good idea, because
this automatic reformatting is bound to do the wrong thing sometimes,
which is very annoying. But I don't see a way to turn it off, so best to
get this change out of the way.

TestSegmentInExcludedCountriesRepair and TestSegmentInExcludedCountriesRepairIrreparable are using 20 storage nodes.
This change make them use 7 by adjusting the test redundancy scheme.

We don't need to have every single test for both, only one for
each should be sufficient. For all other tests it doesn't matter
which one we use.

For nodes in excluded areas, we don't necessarily want to remove them
from the pointer, but we do want to increase the number of pieces in the
segment in case those excluded area nodes go down. To do that, we
increase the number of pieces repaired by the number of pieces in
excluded areas.

inconsistency
The original design had a flaw which can potentially cause discrepancy
for nodes reputation status between reputations table and nodes table.
In the event of a failure(network issue, db failure, satellite failure, etc.)
happens between update to reputations table and update to nodes table, data
can be out of sync.
This PR tries to fix above issue by passing through node's reputation from
the beginning of an audit/repair(this data is from nodes table) to the next
update in reputation service. If the updated reputation status from the service
is different from the existing node status, the service will try to update nodes
table. In the case of a failure, the service will be able to try update nodes
table again since it can see the discrepancy of the data. This will allow
both tables to be in-sync eventually.

If satellite can't find enough nodes to successfully download a segment,
it probably is not the fault of storage nodes.

work
Since we have changed the repair worker to also mark a node as audit
failure if they return a not found error, we should ignore expired
segments when possible

Update repair tests to check if audit score increases for nodes
that successfully send pieces during successfull and failed repairs.

At some point we moved metabase package outside Metainfo
but we didn't do that for satellite structure. This change
refactors only tests.
When uplink will be adjusted we can remove old entries in
Metainfo struct.

Sometimes we see timeouts from DNS lookups when trying to do
repair GETs. Solution: try using node's last IP and port first.
If we can't connect, retry with DNS lookup.

We want to use StreamID/Position to identify injured
segment. As it is hard to alter existing injuredsegments
table we are adding a new table that will replace existing
one. Old table will be dropped later.

This is part of metaloop refactoring. We plan to remove
irreparable at some point but there was not time for it.
Now instead refatoring it for segmentloop its just easier
to drop it.
Later we still need to drop table with migration step.

metabase has become a central concept and it's more suitable for it to
be directly nested under satellite rather than being part of metainfo.
metainfo is going to be the "endpoint" logic for handling requests.

It's impossible to time correctly this check. The segment may expire
just at the time we upload the repaired pieces to new storage nodes.
They will reject this as expired and the repair will fail.
Also, we penalize storage nodes with audit failure only if they fail
piece hash verification, i.e. return incorrect data, but only if they
have already deleted the piece.
So, it would be best if the repair service does not care about object
expiration at all. This is a responsibility of another service.
Removing this check will also simplify how we migrate this code
correctly to the metabase.

Rather than having a single repair override value, we will now support
repair override values based on a particular segment's RS scheme.
The new format for RS override values is
"k/o/n-override,k/o/n-override..."

Make metainfo.RSConfig a valid pflag config value. This allows us to
configure the RSConfig as a string like k/m/o/n-shareSize, which makes
having multiple supported RS schemes easier in the future.
RS-related config values that are no longer needed have been removed
(MinTotalThreshold, MaxTotalThreshold, MaxBufferMem, Verify).

* The audit worker wants to get items from the queue and process them.
* The audit chore wants to create new queues and swap them in when the
old queue has been processed.
This change adds a "Queues" struct which handles the concurrency
issues around the worker fetching a queue and the chore swapping a new
queue in. It simplifies the logic of the "Queue" struct to its bare
bones, so that it behaves like a normal queue with no need to understand
the details of swapping and worker/chore interactions.

What:
Use the github.com/jackc/pgx postgresql driver in place of
github.com/lib/pq.
Why:
github.com/lib/pq has some problems with error handling and context
cancellations (i.e. it might even issue queries or DML statements more
than once! see https://github.com/lib/pq/issues/939). The
github.com/jackx/pgx library appears not to have these problems, and
also appears to be better engineered and implemented (in particular, it
doesn't use "exceptions by panic"). It should also give us some
performance improvements in some cases, and even more so if we can use
it directly instead of going through the database/sql layer.

Sometimes nodes who have gracefully exited will still be holding pieces
according to the satellite. This has some unintended side effects
currently, such as nodes getting disqualified after having successfully
exited.
* When the audit reporter attempts to update node stats, do not update
stats (alpha, beta, suspension, disqualification) if the node has
finished graceful exit (audit/reporter_test.go TestGracefullyExitedNotUpdated)
* Treat gracefully exited nodes as "not reputable" so that the repairer
and checker do not count them as healthy (overlay/statdb_test.go
TestKnownUnreliableOrOffline, repair/repair_test.go
TestRepairGracefullyExited)

Add flag to satellite repairer, "InMemoryRepair" that allows the
satellite to decide whether to download the entire segment being
repaired into memory (this is what the satellite already does), or to
download it into temporary files on disk that will be read from in the
upload phase of repair.
This should help with handling high repair traffic on satellites that
cannot afford to spend 64mb of memory per repair worker.
Updates tests to test repair for both in memory and to disk.

Make sure that suspended nodes are treated appropriately by the overlay
cache. This means we should expect the following behavior:
* suspended nodes (vetted or not) should not be selected for uploading
new segments
* suspended nodes should be treated by the checker and repairer as
"unhealthy", and should be removed upon successful repair
This commit also removes unused overlay functionality.
Fixes a bug with commit 8b72181a1f where
the audit reporter was automatically suspending nodes regardless of
audit outcome (see test added).
Tests:
* updates repair tests to ensure that a suspended node is treated as
unhealthy and will be removed from the pointer on successful repair
* updates overlay tests for KnownUnreliableOrOffline and KnownReliable
to expect suspended nodes to be considered "unreliable"
* adds satellitedb test that ensures overlay.SelectStorageNodes and
overlay.SelectNewStorageNodes do not include suspended nodes
* adds audit reporter test to ensure that different audit outcomes
result in the correct suspended/disqualified states

My understanding is that the nodes table has the following fields:
- `address` field which can be a hostname or an IP
- `last_net` field that is the /24 subnet of the IP resolved from the address
This PR does the following:
1) add back the `last_ip` field to the nodes table
2) for uplink operations remove the calls that the satellite makes to `lookupNodeAddress` (which makes the DNS calls to resolve the IP from the hostname) and instead use the data stored in the nodes table `last_ip` field. This means that the IP that the satellite sends to the uplink for the storage nodes could be approx 1 hr stale. In the short term this is fine, next we will be adding changes so that the storage node pushes any IP changes to the satellite in real time.
3) use the address field for repair and audit since we want them to still make DNS calls to confirm the IP is up to date
4) try to reduce confusion about hostname, ip, subnet, and address in the code base

Previously, we were simply discarding rows from the repair queue when
they couldn't be repaired (either because the overlay said too many
nodes were down, or because we failed to download enough pieces).
Now, such segments will be put into the irreparableDB for further
and (hopefully) more focused attention.
This change also better differentiates some error cases from Repair()
for monitoring purposes.

This new repair timeout (configured as TotalTimeout) will include both
the time to download pieces and the time to upload pieces, as well as
the time to pop the segment from the repair queue.
This is a move from Github PR #3645.
